Pediatric Associates of NYC Vaccine and Visit Schedule
1 month: Hepatitis B
2 month: Diphtheria, Tetanus, acellular Pertussis (DTaP), Polio (IPV), Haemophilus influenzae b (HIB), Pneumococcal vaccine (PCV), Rotavirus (Rotateq)
3 month: Hepatitis B (if not received in hospital at birth)
4 month: DTaP,  Polio, HIB, PCV, Rotavirus (Rotateq)
6 month: DTaP, PCV, HIB, Rotavirus (Rotateq)
9 month: Hepatitis B, Polio
1 year: Measles, Mumps, Rubella (MMR), Varicella (Chickenpox), Hepatitis A & CBC and Lead Screen
15 months: PCV, HiB
18 months: DTaP
2 years: CBC and Lead Screen, Hepatitis A
2 1/2 years
3 years
4 years: MMR, Varicella, Hearing and Vision Screen
5 years*: DTaP, Polio, Hearing and Vision Screen
11 years:Tdap, Menactra
13 years: Gardasil (Requires 2 doses if first dose <15 years of age, at least 6 months apart. Requires 3 doses if first dose given >15 years of age, 2 and 6 months after first dose))
14 years: Gardasil
16-17 years:  Menactra
18-19 years MenB (Trumenba) (requires 2 doses separated by at least 6 months)
*Visits after the age of five continue on an annual basis. We follow the American Academy of Pediatrics Guidelines for the timing of vaccinations.
